Registration
Registration for the upcoming school year 2006/2007 is underway at Decker. All returning and new parents are asked to call the school at 330 780-3208 ext. 0 to schedule an appointment for registration before June 2, 2006. After this date registration will begin August 14, 2006. Registration packets may be picked up at the Decker building throughout the summer months. Items you will need for registration are;
- Child’s birth Certificate
- Shot record
- Income verification
- Proof of health insurance
- Social Security card
- Parent/guardian Social Security number
- Proof of residency in Barberton or Norton
- Names address and phone #’s of 3 emergency contact people
- Doctor and Dentist address and phone #’s
